Shutdown and recommissioning record
A record for managing systems through closure or low-use periods and safely bringing them back into use - covering the action taken at shutdown and the flushing, disinfection and checks done before reopening.
Print-ready PDF with the columns laid out, or the CSV to fill in on a computer.
| Date | System / area | Stage | Action taken | Flush/disinfect done | Temps/residual OK | Sample (Y/N) | Sign-off |
|---|---|---|---|---|---|---|---|
| 2026-04-01 | Whole building | Shutdown | Drained little-used areas; flushing plan set | N/A | N/A | N | FM |
| 2026-06-20 | Whole building | Recommission | Full flush + thermal disinfection | Y | Y | Y | FM |
Sample rows are illustrative. Download the PDF for a clean print-ready form, or the CSV to fill in digitally.
How to use it
- Record your plan for closed or low-use periods - either maintain flushing and temperatures or drain down systems to control stagnation.
- Before reopening, flush thoroughly, disinfect where the risk assessment indicates, and confirm temperatures and any disinfectant residuals.
- Sample before return to use where the risk assessment or duration of closure justifies it, and review the risk assessment after prolonged closure.
- Sign off each system as fit to reopen; reference HSG274 and current shutdown/reopening guidance rather than assumed periods.
