TMV service and fail-safe record
A record for servicing thermostatic mixing valves - checking the fail-safe shutoff, cleaning/descaling strainers and disinfecting - balancing scald protection with legionella control.
Print-ready PDF with the columns laid out, or the CSV to fill in on a computer.
| Date | TMV ID / location | Blend temp °C | Fail-safe OK? (Y/N) | Serviced? (Y/N) | Disinfected? (Y/N) | Engineer |
|---|---|---|---|---|---|---|
| 2026-06-14 | TMV-204 (shower) | 41 | Y | Y | Y | Competent contractor |
| 2026-06-14 | TMV-118 (basin) | 43 | N | Y | Y | Competent contractor |
Sample rows are illustrative. Download the PDF for a clean print-ready form, or the CSV to fill in digitally.
How to use it
- Service TMVs at the interval set by your risk assessment and the manufacturer, recording the delivered blended temperature.
- Test the fail-safe so the valve shuts off flow if the cold supply fails, and note pass/fail.
- Clean or replace strainers and filters, descale, and disinfect components, recording each step as TMVs sit at growth-permissive temperatures.
- Log any failed valve as an action for repair or replacement and re-test after work; verify targets against HSG274.
